Goal One: To provide KY citizens with free and open access to federal government documents.

OUTCOMES:

1. Depository patrons can locate pertinent sources of government information at, or close to the time of initial inquiry.

Assessment procedure:
Record time of transaction on reference query sheet.

Criteria for assessment:
Lapsed time averages ____ minutes.

2. Patrons will be able to locate a significant portion of their requested information from the respective depository's collections and Internet-based resources.

Assessment procedure:
Patron responds favorably to librarian's exit question on success of search.

Librarian records response on reference tally sheet.

Criteria for assessment:
___ % of patrons report search as "successful."

3. Depository patrons with disabilities will be satisfied with the level of physical access to the government documents collection.

Assessment procedure & criteria:
On a patron survey, ___ % of individuals with disabilities rate document access as "Satisfactory" or higher.

Goal Two: To provide efficient and instructive reference assistance designed to make FDL patrons self-sufficient.

OUTCOMES:

1. Patrons will be satisfied with the depository's level of reference assistance services.

Assessment procedure:
Patron verbalizes satisfaction in response to librarian's exit question.

On a patron survey, respondent rates level of reference assistance as "Satisfactory" or higher.

2. Document patrons will be able to demonstrate search skills that are clearly observable by depository personnel.

Assessment procedure:
Record and tally number of repeat searchers who decline assistance.

On a survey, patron rates as "Satisfactory" or higher the learning tools (guides, instruction, indexes, etc.) made available by depository personnel.

Criteria for assessment:
___ % of patrons are satisfied with available learning tools.

Goal Three: To strengthen interlibrary reference cooperation among KY's depository libraries.

OUTCOMES:

1. Document patrons will be satisfied with their ability to receive government information through Internet portal services e.g., KVL and the UK Regional Depository Gateway.

Assessment procedure & criteria:
In response to the librarian's reference transaction sheet query, ___ % of depository patrons indicate their awareness of KVL and RD Gateway services for access to and retrieval of government information.

On a survey of depository patrons, ___ % of respondents report satisfaction with their ability to receive government information through the portal services.

2. Patrons who make use of ILL services will report satisfaction with the timeliness of ILL request fulfillment.

Assessment procedure & criteria:
On a patron survey, ___ % of respondents who require ILL services report satisfaction with the timeliness of ILL request fulfillment.

Goal Four: To provide appropriate and functioning equipment for use of materials within the depository library, including accessing, viewing, copying, printing, and downloading functions.

OUTCOMES:

1. Depository patrons will be able to use government documents on functioning equipment.

Assessment procedure & criteria:
On a survey of patrons, ___ % of respondents will report depository equipment function as "Satisfactory" or higher.

2. Patrons will report satisfaction with the depository's reproduction facilities and equipment.

Assessment procedure & criteria:
On a visual check at opening time, depository staff can confirm that at least one of each type of documents equipment (copier, printer, Net browser, CD drive, etc.) is operable.
