Registration and Library Orientation


Upon initial contact with the Medical Center Library (MCL), the individual with a disability will be asked to complete a Disability Services registration form.

  • If the individual is unable to complete the form, a MCL staff member will provide assistance. This form contains space for basic contact information and a checklist where the disabled individual may indicate his/her special needs.
  • This form will be filed in the Disability Registration Notebook at the MCL User Services Desk and will be referred to as needed by MCL staff in order to provide continuous and seamless service to the individual.

 
The Medical Center Library will provide orientation to the Library and instruction in the use of the Library's resources.

  • The Contact Person (CP) may provide this service.
