Meeting Room Request Form

 

TO BEGIN YOUR REQUEST, PLEASE CHECK THE HOURS OF OPERATION TO MAKE SURE THE BUILDING WILL BE OPEN AT THE TIME(S) YOU NEED IT, BEARING IN MIND THAT ROOMS WON'T BE AVAILABLE ON ANY DAY UNTIL THE BUILDING HAS BEEN OPEN FOR AT LEAST 30-MINUTES.  NEXT, READ THE MEETING ROOM POLICIES AND FILL OUT THIS FORM.

 

Please include all of the information requested. Place your cursor in the first box, enter the information needed, then tab to the next box, etc.  When all boxes are complete, hit the "Submit" button at the bottom of the form.

Select seating style from choices below.

Small Meeting (6 or 8 tables in a rectangle with chairs seating 24 or 32)

Open Meeting (5 or 7 tables in a U formation with chairs seating 20 or 28)

Reception (four tables together in middle of room with chairs on room periphery; seats up to 50)

Theater (six chairs in a row with an aisle down the middle; seats up to 50)
